Hydrogen Water

Alkaline ionized water, known in science as electrolyzed reduced water. Its key ingredient is dissolved hydrogen. For the first time, everything about it is gathered in one place: history, research, myths and safety. Every claim comes with a source and a label showing how well it has been tested. The water we came from

Nature "tuned" water long ago. The ocean is slightly alkaline, a baby grows for nine months in slightly alkaline water, and almost everything inside us is slightly alkaline too. Only the "entrances" are acidic: the stomach, the skin, the vagina. There, acid acts as a guard.

Seawater is slightly alkaline. Because of carbon dioxide, its pH has fallen from 8.2 to 8.1 since the start of the industrial era. The ORP of seawater is positive, about +430 mV: dissolved oxygen makes it so.

The cell as a spaceship

A cell is sealed by a membrane, and water enters through special airlocks: aquaporins. They were discovered by the American Peter Agre in 1992, earning him the 2003 Nobel Prize in Chemistry. An aquaporin lets water through and holds back charged particles. A single kidney has tens of billions of these channels: the kidneys filter about 170 litres of primary urine a day and return almost all of it to the blood, excreting about a litre. We have forgotten how to drink

Many people drink only when thirsty. But thirst switches on when the body has already lost 1–3% of its water, and in older people it is blunted. So it is better to drink water on a schedule rather than by thirst. Jéquier, Constant, 2010 ↗

How water becomes "living"

Inside an ionizer, water flows between two electrodes separated by a membrane. Each electrode produces its own kind of water.

Ionizer diagram: the cathode gives alkaline water with hydrogen, the anode gives acidic water
Illustration

How an ionizer works. The membrane lets only ions through and separates the alkaline water (catholyte) from the acidic water (anolyte). For drinking, alkaline water at pH 8.5–9.5 and no higher than 10 is recommended.

The pH scale: where each water sits

Illustration

What it is called. In Russian: alkaline ionized, "living" or hydrogen water. In English: alkaline ionized water, scientifically electrolyzed reduced water (ERW), and in Japan increasingly electrolyzed hydrogen water. All of these are the same water from the ionizer's cathode.

ORP, oxidation-reduction potential, shows whether water takes electrons or gives them away. Plus means the water oxidises, minus means it reduces. Ionized water gets its negative ORP from dissolved hydrogen, not from the alkalinity itself. This is the conclusion of a 2022 review.

Hydrogen escapes, so ionized water is drunk fresh. The Japanese manufacturers' association warns that during storage and when making tea, pH returns to neutral.

How to choose a bottle

Look for

  • Electrodes: platinum-coated titanium.
  • A PEM/SPE membrane or independent testing for chlorine and ozone.
  • Hydrogen concentration in mg/L (ppm), measured by an independent laboratory.
  • A flask of glass or BPA-free food-grade plastic.

Be careful

  • "Active" or "atomic" hydrogen are advertising words. What works is molecular H₂.
  • A smell of chlorine after the cycle is a reason not to drink that water.
  • ORP alone does not show how much hydrogen is in the water.

Reactors, minerals, nature

A kitchen ionizer is not the only way to get alkaline water with a negative ORP. There are industrial reactors, magnesium and mineral cartridges, and natural products that change water.

Mineral ionizers: magnesium and cartridges

Magnesium gives electrons to water molecules, forming molecular hydrogen H₂ (Mg + 2H₂O → Mg(OH)₂ + H₂). It is dissolved hydrogen that makes ORP negative. The ORP values at the bottom are approximate: every water is different. Hydrogen escapes over time, so this water is drunk fresh.

2010–2011Trials

A magnesium stick makes hydrogen water

Metallic magnesium reacts with water and releases hydrogen, and the water becomes more alkaline. This is exactly how water was prepared in human trials: a magnesium stick gave about 0.55–0.65 mM of hydrogen. Trials in metabolic syndrome and during radiotherapy of liver tumours used this water.

07 · Path of water

What happens after a sip

We compare, step by step, acidic and reverse-osmosis water with alkaline ionized water. Where there is no difference, we say so.

Leonardo-style illustration: pH in the body's organs and fluids
Illustration

Each organ has its own pH. The values are approximate; the stomach (1.5–3.5) and blood (7.35–7.45) are measured precisely.

Stage
Acidic and reverse-osmosis water
Alkaline ionized
StomachpH 1.5–3.5
It simply dilutes gastric juice. The stomach is thousands of times more acidic than this water, so the slight acidity of osmosis water means nothing to it.
Briefly raises stomach pH. This is the basis of the official Japanese effect "for hyperacidity". In a test tube, water at pH 8.8 irreversibly switches off pepsin, the enzyme that burns the oesophagus in reflux.
Intestineabsorption
Absorbed like ordinary water. Water after osmosis brings almost no calcium or magnesium.
Officially recognised in Japan and Korea: helps with diarrhoea, indigestion and fermentation. In a 2021 trial, 9 out of 10 people had more bifidobacteria and their stools normalised.
BloodpH 7.35–7.45
Blood pH does not change: the kidneys and lungs hold it steady.
Blood pH does not change noticeably either: even a change of diet shifts it by about 0.01. But in athletes, three weeks of alkaline water increased buffer reserves (bicarbonate) and lactic acid cleared faster.
Cellshydrogen H₂
No hydrogen.
Hydrogen, the smallest molecule, passes through cell membranes. In experiments it neutralises the most aggressive radicals. In human trials, markers of oxidative stress fell.
Kidneysexcretion
They remove the excess, and urine stays as usual.
Urine becomes more alkaline: this is the "alkalisation" you see on test strips. With kidney failure this water must not be drunk.

The bottom line on "alkalising the body". Alkaline water temporarily changes stomach pH, urine pH and buffer reserves. It does not change blood pH, and that is a good thing: the body keeps it within narrow limits. Sources: Fenton, Huang. BMJ Open, 2016 ↗ Chycki et al. PLoS ONE, 2018 ↗ Koufman, Johnston. 08 · Acidosis

Acidification: what is true

Doctors call it acidosis when blood pH falls below 7.35. It is an emergency, not a lifestyle. But scientists also have a milder, well-described concept: dietary acid load. This is where water can help.

Acid load

Meat, cheese, fish and grains add acid to the body; vegetables and fruit add alkali. With a high load, urine has more calcium and less citrate, and the risk of stones rises. In some people kidney disease progresses faster.

Water lowers the load

In a trial in 129 people, 1.5–2 L of bicarbonate mineral water a day for 4 weeks reduced acid excretion by the kidneys by 48–68%. In athletes, alkaline water increased buffer reserves.

Wasserfurth et al., 2019 ↗ Chycki et al., 2018 ↗

What is important to understand. Water lowers the acid load through bicarbonate and minerals. Blood pH does not change; the kidneys and lungs hold it. As for the idea that bones give up calcium to neutralise acid, the data on bone density in people on an "acidic" diet are contradictory. Osuna-Padilla, 2019 ↗

11 · Blood sugar

Blood sugar

There is a signal in several small trials, mostly with hydrogen water. There are no large trials yet. The Japanese association states plainly that an effect in diabetes is not officially recognised.

Strength of evidence

Promising, but few people and short durations. The water can be drunk alongside treatment, but not instead of it.

Why blood sugar may fall

Hypothesis

Less oxidative stress and inflammation

Hydrogen lowers inflammation markers (TNF-α, IL-6, CRP). Inflammation stops insulin from working.

Hypothesis

Gut bacteria

In a 2023 trial, hydrogen water changed the make-up of the microbiota, and these changes were linked to metabolism.

Hypothesis

Simply more water

When water is short, vasopressin rises, and it raises blood sugar. People with an ionizer usually start drinking more.

15 · Osmosis

Osmosis and acidic water

Reverse osmosis removes almost everything from water: both contaminants and calcium and magnesium. This water is clean, but empty.

Czech hygienist František Kožíšek reviewed the risks of drinking demineralised water long-term for a report prepared by the WHO. The body gets too little calcium and magnesium, and soft water leaches more metals from pipes. That is why water after osmosis is usually remineralised. Kozisek. Health Risks from Drinking Demineralised Water ↗

The slightly acidic reaction of osmosis water comes from dissolved carbon dioxide and is not dangerous in itself. The main drawback of this water is that it contains almost no minerals.

Acidic water from the ionizer (anolyte) is not for drinking. It is used for washing your face and dishes. 3aaa, questions and answers ↗

17 · Safety

How to drink it, and who should not

Rules from the recommendations of the Japanese manufacturers' association, the Korean regulator and the Mayo Clinic.

How to start

  • For the first two weeks, one glass a day at pH 8–8.5.
  • Then pH 9–9.5. The amount tested in Japan is 0.5–1 L a day.
  • In Korea, about pH 9.5 and no higher than 10 is recommended.
  • Drink it fresh: during storage hydrogen escapes and pH falls.

Medicines and your doctor

  • Take medicines with ordinary purified water.
  • If you have a chronic illness or are pregnant, consult your doctor.
  • If you feel worse or your symptoms do not go away, see a doctor.
  • With diabetes and hypertension, water does not replace treatment. Keep monitoring your blood sugar and blood pressure.

Do not use

  • With kidney failure or impaired calcium excretion.
  • With achlorhydria, when the stomach does not produce acid.
  • For children under one year, including for infant formula.
  • Acidic water from the ionizer, internally.

pH too high

  • Above pH 9.8, excess potassium in the blood is possible, especially with kidney disease.
  • Children and older people should start with pH 8–8.5.

18 · Myths

Myths and reality

There are myths on both sides: among sellers and among those who think ionized water is useless.

A huge wave and lightning in a stormy sea
Illustration
Myth

There is no research; it is just marketing.

Reality

The ionizer has been a medical device in Japan since 1965, and in Korea too. There are double-blind trials on the gut, blood sugar and sport, and data on dialysis. An Indian systematic review in 2022 found noticeable benefit in small trials. Review, 2022 ↗

Myth

In Japan, ionized water is an official cure for everything.

Reality

In Japan the ionizer is a registered medical device. Its effect on the stomach and gut is officially recognised. The manufacturers' association calls effects in diabetes and atopic dermatitis unrecognised. 3aaa ↗

Myth

In the USSR, "living water" was prescribed as a medicine.

Reality

The method was officially recognised by the USSR Higher Attestation Commission in 1985. It was "dead water" that became a medicine: anolyte is registered as an antiseptic. Drinking catholyte was used experimentally in Soviet medicine; we found no document registering it as a medicine.

Myth

Alkaline water alkalises the blood.

Reality

Blood pH stays within 7.35–7.45. What changes is urine pH, stomach pH for a while, and buffer reserves in athletes. BMJ Open, 2016 ↗

Myth

A negative ORP is down to alkalinity.

Reality

A negative ORP comes from dissolved hydrogen. It escapes, so freshness matters. LeBaron et al., 2022 ↗

Myth

The higher the pH, the healthier.

Reality

For drinking, 8.5–9.5. The Korean regulator advises against going above 10, and the Mayo Clinic sees a risk of excess potassium above 9.8.

Myth

Alkaline water cures cancer.

Reality

A systematic review found not a single human study in which alkaline water prevented or treated cancer. BMJ Open, 2016 ↗

Myth

Water after osmosis is the healthiest because it is the purest.

Reality

It is the purest, but it has no calcium or magnesium. If you drink it all the time, it is better to remineralise it. Kozisek ↗

"I gathered everything in one place because water has many opponents and many supporters. The opponents are most often those who have never drunk this water. The supporters know what hydrogen water with molecular hydrogen is, and alkaline, acidic and silver water, and they use it for their health and the health of their loved ones."
